Patient Information & Service Terms

Orders & Documentation

Mr. Vein LLC is responsible for drawing and delivery of laboratory specimens, except where a lab kit arrangement specifies otherwise. Laboratory specimens are not collected without a physician order or an eligible laboratory request.

Before a scheduled blood-draw appointment, Mr. Vein LLC needs the requested patient information, lab order/requisition, identification, and insurance information when applicable. Missing required documentation may result in cancellation.

EKG Services

Mr. Vein LLC performs 12-lead EKG and portable EKG testing. Mr. Vein LLC does not diagnose patients, prescribe treatment, provide medical advice, or interpret results as a physician.

Patients are responsible for sharing EKG information with their physician or other qualified healthcare professional when appropriate.

Specimen Delivery & Laboratory Responsibility

Mr. Vein LLC handles delivery of applicable specimens to the appropriate laboratory. When courier pickup is required, Mr. Vein LLC is not responsible for delays after a specimen has been delivered to or accepted by the courier or laboratory.

Once specimens are received by the laboratory, processing is the laboratory's responsibility. Laboratory billing and outstanding balances are also separate from Mr. Vein LLC service fees.

Payment, Privacy & Cancellations

Payment for Mr. Vein LLC services is collected after the blood draw or EKG unless other arrangements apply. Mr. Vein LLC is an independent convenience service and is not affiliated with laboratories or doctor's offices.

Client information is kept private and is not intentionally disclosed to unauthorized personnel. Please provide 24-hour notice when possible if an appointment must be cancelled.

Important: By using Mr. Vein LLC services, the patient or responsible party acknowledges the applicable service fees and agrees to the service terms for the scheduled blood draw, EKG, portable EKG, specimen pickup, lab kit, or self-order service. Mr. Vein LLC does not diagnose patients, prescribe treatment, provide medical advice, or interpret results as a physician.
