2. Let us know your preferred lab (i.e. Quest Diagnostics, Labcorp, UCI, etc.).

 

3. Email us your *lab order, *insurance card, and *ID card. (If you don’t have an order your provider can fax it). (*Required documentation).

FAILURE TO PROVIDE REQUESTED DOCUMENTATION, APPOINTMENT WILL BE CANCELLED.

 

Mr. Vein charges a service fee of $125. The fee includes venipuncture, specimen processing, and delivery.

 

(This price is for patients within 30 miles of Buena Park, CA 90621. For patients beyond 30 miles the service fee is $150. No service beyond 50 miles).

 

$100 service fee for patients with kits and/or Autologous Serum Eye Drops (With tubes and order). (This price is for patients within 15 miles of Buena Park, CA 90621. For patients beyond 15 miles the service fee is $125. $150 for patinets beyond 30 miles. No service beyond 50 miles).

 

$50 specimen pick-up includes processing & delivery.
